Safety Plan Field Form

  • It is suggested that each manager print out 3 copies of each of the Accident and Field Safety forms.
  • For each observation of a field safety or maintenance issue fill out a Field form.
  • Hand the form into the Snack Stand.
  • These forms will be collected by the Maintenance Director.
  • The Maintenance Director will follow up with the Manager and others to take required action.
