Safety Plan Field Form

- It is suggested that each manager print out 3 copies of each of the Accident and Field Safety forms.
- For each observation of a field safety or maintenance issue fill out a Field form.
- Hand the form into the Snack Stand.
- These forms will be collected by the Maintenance Director.
- The Maintenance Director will follow up with the Manager and others to take required action.

- For each injury incident fill out an Accident Form.
- Hand the form into the Snack Stand.
- These forms will be collected by the Safety Officer.
- The Safety Officer will follow up with the Manager, Player Family and others to take required action.
